Lab Affiliations
Our office is equipped with a full service outpatient lab. We have partnered with LabCorp to be able to offer you lab services directly in our office rather than traveling to another location for your lab tests.
If your insurance plan requires you to use a different out patient lab service you MUST NOTIFY the office prior to your labs being completed.

Thomas Hospital’s newly re-modeled labor and delivery unit offers family-centered care, compassionate and supportive staff, a special care nursery with a neonatologist on staff – and a warm welcome to every new family. They offer a variety of childbirth and baby care classes, a breast feeding class and a lactation consultant. They even offer a free car seat to get your new arrival home safely.
Office Administration
Please prepare for your appointment by bringing:
- Photo ID* (If address on ID does not match address in our records, other proof of identification must be shown)
- Health Insurance Card/Information
- A friend or family member to take notes/help with questions
- A list of all medications you are taking
- A list of your questions/concerns
- Copies of records from doctors related to your visit with us
- A parent or legal guardian must accompany any minor patient (under age 18) for the first visit. Accompaniment for subsequent visits will be discussed during the initial visit.
- Plan to arrive 15 minutes early to fill out paperwork unless you do this ahead of time under the “FORMS” tab and bring them with you!
To protect our patients’ identities and to comply with Federal Law, a photo ID and health insurance card MUST be shown at check-in. Failure to provide the required documents will result in your appointment being cancelled.
Scheduling your appointment:
The nature of our practice is to give our patients the utmost in care and service. Please excuse any delays. We will do our best to see you at your designated appointment time as we recognize your time is valuable. We will give you the same careful attention as soon as possible.
As a courtesy to other patients and staff, please call the office as soon as possible if you are unable to keep your appointment or are going to be late. We may be able to reschedule for another day.
Contacting Us After Business Hours
After hours, our answering service will contact the provider on call. Please do not hesitate to contact the provider for an EMERGENCY situation. Providers DO NOT refill medications or give laboratory results. Requests of this nature need to be made during regular business hours.
Non-urgent Medical Messages
These messages are taken by our office staff and forwarded to the providers and are answered at the end of office hours.
Medical Records
All requests for medical record copies must be placed in writing and include your signature. You may either mail your request to our office or fax your request to 304-766-4337 Attention Medical Records. There is also a charge of $10.00 plus .75 cents a page that must be paid before records are sent.
