What do I need to bring to my first appointment?

Please bring a valid photo ID, your health insurance card, a complete list of your current medications, and any relevant medical records from previous providers. If your photo ID address does not match our records, additional proof of identification will be required. To comply with federal law, a photo ID and insurance card must be shown at check-in.

Yes. All minor patients under the age of 18 must be accompanied by a parent or legal guardian for their first visit. Accompaniment requirements for subsequent visits will be discussed during that initial appointment.
